Anyone who has been on a diet knows, it can be super hard to get rid of stubborn fat.

“Though exercise can help correct a metabolism that’s been out of whack for a long time, the grisly reality is that it may not ever go back to what it was before you gained weight,” reports Alice Walton, writing for Forbes. “So if you’ve been overweight or obese and you lose weight, maintaining that loss means you’re probably going to have to work harder than other people, maybe for good.”

The National Weight Control Registry found long-term weight-loss maintenance can require sustained increasing levels of physical activity. Unfortunately, that might not be an option for everybody.

For long-term dieters who have been unable to eliminate fat from their bodies, there are some medical options, including a recently approved procedure that freezes fat cells.

These options include:

Weight loss surgeries

Several different surgical methods are effective at helping people lose weight. Most involve reducing or restricting the stomach size or limiting the digestive process. They include gastric bypass, gastric band, gastric sleeve, duodenal switch or electric implant. WebMD.com explains after weight loss surgery, most people lose weight for 18-24 months. At that point, many people start to regain some of their lost weight.

Side effects and complications, including death, are possible with any type of surgery. With bariatric surgeries, the most common side effects include nausea, vomiting, bloating, diarrhea, excessive sweating, increased gas and dizziness. Serious problems can include bleeding, infection and blood clots in the legs that can move to the heart and lungs.

Best for: People who have a body mass index (BMI) of 40 or higher, so roughly 100+ pounds overweight; those who have a serious health problems related to obesity; or people who have been unsuccessful losing weight by other means.

Liposuction

This process uses suction to remove fat cells from specific areas. A doctor makes incisions to insert a vacuum tube which is moved around under the skin as needed. Although it is an outpatient procedure, patients usually miss a week of work but swelling, bruising and soreness can last for several weeks.

Contrary to popular belief, cellulite cannot be removed with liposuction, and skin can sag and be loose following removal of fat deposits.

“Most patients are pleased with the outcome of their liposuction surgery. However, like any other medical procedure, there are risks involved,” explains the Food and Drug Administration. “That’s why it is important for you to understand the limitations and possible complications.”

The agency lists a full page of potential side effects, some of which are pretty serious.

Best for: Adults within 30 percent of their ideal weight who have firm, elastic skin and good muscle tone.

CoolSculpting

Approved by the FDA in 2014, CoolSculpting is a safe and effective method for permanently eliminating fat cells in specific body locations. During an in-office procedure, fat cells under the skin are cooled and they die. Over the following weeks, the body then gets rid of those cells in a natural process. There is no pain.

Best of all, side effects from CoolSculpting are virtually nonexistent. According to the FDA, “adverse events reported during the studies included numbness and mild contour irregularity. All adverse events but one resolved by the 16-week follow-up.”

According to the FDA, “Clinical testing demonstrates that the CoolSculpting system can cause lipolysis with a subsequent removal of fat. Clinical results for the flank, abdomen and thigh, support the safe and effective use of the device for this indication.”

In other words, freezing your fat away is a legitimate option for people who want to reduce fat in specific areas.

Best for: People who are within 35 pounds of their weight-loss goals and have not had luck getting rid of fat through diet and exercise.

Let’s face it, it can be downright discouraging to work hard and eat right and still have to deal with stubborn fat. Thankfully, there are finally options that actually work.
