Thousands of Arizonans hit in Medicaid agency’s data breach

PHOENIX — Thousands of Arizonans were affected by a data breach earlier this year that targeted the state’s Medicaid agency, it was announced Monday.
The breach of the Arizona Health Care Cost Containment System affected more than 3,100 individuals when their IRS 1095-B forms were delivered to the wrong addresses.
The agency is required to send the forms to its 1.8 million members.
Spokeswoman Heidi Capriotti told KTAR News 92.3 FM an internal review found that a programming error led to the mix-up.
The forms do not contain social security numbers, but do have names and dates of birth.
Capriotti said the agency is informing those impacted by the breach on Monday.
Anyone who was affected was advised to place a free credit report fraud alert or a credit alert security breach on their account, request a credit report or report any suspected identity theft.
