Patients at 2 closed Valley hospitals have until June to request records

PHOENIX — Patients at two Phoenix-area hospitals that closed their doors last year have a few more months to request and obtain copies of their medical records.
Anyone who used to seek care at Gilbert Hospital or Florence Hospital at Anthem will have until June 23 to get the records under a court-approved plan.
After that date, all of the medical records will be permanently destroyed for security measures. The hospitals closed last June due to financial issues.
Patients can get their records by filling out a medical record request form for either Gilbert Hospital or Florence Hospital at Anthem and emailing it to nvhrecords@resolutecommercial.com.
They can also call 480-947-3173 or mail Resolute Commercial at 7201 E. Camelback, Ste. 250, Scottsdale, Arizona.
Once the requests are processed and applicants are notified, medical records can be picked up at the Resolute Commercial offices.
Medical records can also be requested on behalf of a patient, but documentation to validate their ID, such as a power of attorney, must be presented at the time of the request and pickup.
